To conduct a ship survey, the following forms and sheets are required and used by surveyors to conduct a survey and they must be completed in full details:

1. The forms S.2, S.3, S.4 of Report by Surveyor on Completion of Survey or Inspection,
2. The Master Sheets (for Machinery, Equipment, Hull & Load Line)
3. The Radio Survey Sheet or form &
4. The Ship Survey checklist
